She is an Indian film and television actress.  She received a nomination for the Filmfare Award for Best Supporting Actress for her role in the movie Akele Hum Akele Tum (1995). She agreed to participate in Sanjay's Leela Bhansali film, Bajirao Mastani, playing Radhabai, mother of Bajirao I in 2014. She had to go bald for her role in the film. She received the National Award for Best Supporting Actress for her outstanding performance in the movie Bajirao Mastani.

Azmi portrayed a doctor in trouble in the TV series, Jeevanrekha and a young widow in the TV movie Rao Saheb (1986) directed by Vijaya Mehta. She also starred in the Malayalam-language film Vidheyan (1993), directed by Adoor Gopalakrishnan Azmi is married to Baba Azmi, director of photography and brother of Shabana Azmi, thus linked to the family of Akhtar-Azmi films.Her niece, Saiyami Kher, debuted at Bollywood in the 2016 film Mirzya.
